York College in the High School Program: Student and Parent/Guardian Agreement
By completing and sending in this application for the York College in the High School program, the student and their parent or legal guardian agree to what's written below regarding the college-level course(s) taken at the high school.
Sharing Your School Information: The student gives York College permission to share any and all information about their academic record and grades, specifically for the College in the High School course(s) with their high school. The student also understands that York College is not responsible for any problems that might happen because this information is shared.
Paying for the Program: Both the student and the parent or legal guardian understand that students in this program cannot get college financial aid (like grants or loans to help pay for college). The student understands and agrees that paying the cost for this class is their responsibility. Both the student and parent/legal guardian also understand that the extra costs for things like required books or materials for the course are the student's responsibility. Furthermore, it is understood by both the student and parent/legal guardian that if there are any extra costs from trying to collect payment or any legal fees related to the course charges, the student will be responsible for those too. By sending in this application, the student understands they are agreeing to pay for the class credits and that they still have to pay even if they stop attending the class in the high school.
Dropping the Class: The student knows that if they want to stop taking the college-level class, they must officially withdraw through the process provided by York College and their high school. The student understands that they will not get their money back if they officially withdraw after the refund deadline. The student also understands that if they decide to withdraw, they must inform their high school counselor.
College-Level Coursework: The parent/legal guardian gives permission for their son/daughter to enroll in this college-level course offered through York College at their high school. They understand that the academic expectations and material in this course may be more rigorous than typical high school classes.
Getting College Credit Somewhere Else (Transcripts): The student understands that if they want to use the credit earned in this course at another college later, they will need to ask for and pay for an official report of their York College grades (called a transcript) from the York College Records Office to be sent to the other college. The student understands that it is up to the other college to decide if they will accept the credit from this class.
Confirming Your Information: The student promises that all the information they gave in their application is true and correct.
By submitting this form online, both the student and the parent/legal guardian are saying that they have read, understand, and agree to everything written here about participating in the College in the High School program at their high school.
