Informed Consent- Graham Collaborative Innovation Fellows Program

The learning survey you are completing contains a number of questions that can help us to gauge the habits of mind innovative students bring with them to college. To help us to better understand the kinds of students who succeed in the Graham Collaborative Innovation Fellows Program, we’d like to keep some of the information that we gather in the admission process and use it for program improvement. We might also share what we learn with other scholars through publication and presentations.

All of the information collected will be confidential. Your name will not be associated with the responses.

You are not required to give your consent, and whether you consent or not will have no influence on inclusion in the scholar program for which you applied. The scholar program review committee will not know whether you’ve consented or not.
